It is important to obtain your response, because only faculty can provide an assessment of their needs, interests, and preferred methods of support in integrating technology into the classroom. This data will be used in planning for providing both resources and training over the next few years.

By making creative use of a general purpose answer sheet (form # 16504), we hold costs and processing time to a minimum. You may use pen or pencil to complete the form. This form may be completed anonymously. We are using the bubbles in the Last Name section of the form to separate responses by university and academic unit. Side 2 of the form contains a red square with the admonition, "Do Not Write in This Space." We've programmed the scanner to permit you to write in that space, so please do so if you wish to supply information not covered in the survey. If you wish to provide further input regarding teaching with technology, then provide your name, campus address and phone in the red square. Someone will contact you. Please do not fold the form when you complete it. Place it in an 8 x 12 envelope and return it via campus mail to your campus representative on or before Wednesday, February 21, 1996.
